Please indicate if you are scheduling as a New patient (NP) or existing/follow up visit. PLEASE NOTE THERE IS A NO SHOW FEE FOR NEW & EXISTING PATIENTS. As a courtesy to other patients and our doctors, please be sure to promptly cancel/ reschedule your appointment.
The MEDENT Patient Portal provides secure access to your medical records along with contact to staff and providers. Please be sure to use all caps for your username upon login. The password is case sensitive.
You are now able to schedule an evisit through your patient portal as well as schedule an appointment in the office.
